Specify M4b (approvals over Mattermost); propose P16 on approval replies

The owner's decisions of 2026-09-24: commands only, no reactions; an approvers list that falls
back to allow.users; approvals in the asking thread; on a gatewayd restart, old posts are marked
dead and brokerd's courier posts pending approvals again.
