kyleandGrok 4.6 4dd3b87db3 docs: M5d-2 task 06: cp first, tick is glue
The second 06 session listed tests/, read eight loopd modules and
support/mod.rs, grepped bxcore, then hit Context size exceeded with
no commit. The cp is now the first action; tick is glue over two
todo!() bodies; run-plan tells the session to run step 1 before
reading crate source.

kyleandGrok 4.6 2eb0ffd6d7 docs: M5d-2: read only the implementer-log header
Task 06 opened the whole implementer log (~360 kB) after reading supporting
modules; OpenCode returned "Context size has been exceeded" and made no
commit. AGENTS.md and tasks 06-10 now say to read the first 12 lines only.

kyleandGrok 4.6 2a48b7f36d docs: M5d-2: after gate: ok, do not sum test results
Task 01 told Ornith to add up every `test result:` N passed. A greedy
sed kept the last digit (1512 became 1022) and it stopped with the
correct T1 and given test uncommitted. T50/T59: the last line of
make gate is the count; summing is not a stop.

kyleandClaude Opus 5.5 161b16353b docs: the M5c-1 review, tips T50 to T58, and AGENTS.md rules from the run
The review of tasks 01 to 15 in the implementer log, with the six findings fixed in review. Nine
tips for writing tasks from the run's transcripts and the review, new evidence for I16 and I17,
and four rules in AGENTS.md: the Cargo registry is outside the repository, `git stash -u` and the
README's flaky tests, an edit never anchors on another line, and what a test count includes.

kyleandClaude Opus 5.5 1fb0ec8a06 docs: the M5b review, and tips T45 to T48
The review of tasks 01 to 14 in the implementer log (task 14's row
renamed to its file's name), and four tips for writing tasks: what to
do with a skeleton's comments, golden files for scanners, naming the
log row, and running the implementer without plugins.

kyleandClaude Opus 5.5 224617ee65 docs/plans/CHECKLIST.md: the checks before a plan is handed over; T41 to T43
One list for the design model to work through per task, gathering the
task-writing tips that apply before hand-over (T18, T19, T24 to T29,
T32 to T35, T39, T40) and three new ones from the combined Ornith
guidance: walk each given test by hand against the task's rules (T41),
grep earlier given files for anything the task changes (T42), and run
the gate's own checks on the given files (T43).

kyleandClaude Opus 5.5 e41a204902 M5a review: tasks 01 to 22 and the device check; tips I16, I17, T34 to T40
Two review entries in the implementer log: tasks 01 to 15 (the header
forgery, review ids, the runbook entries, log rows written from memory)
and tasks 16 to 22 with the device check on straylight (the ledger
raising state by the grant's label, the note answers, the stale target/
that stalled task 21, the clippy 1.95 stop, the fake podman race). Two
short rules go into AGENTS.md: stop after one check when a test outside
the task fails, and write the log row from the diff.

kyleandClaude Opus 5.5 fb0aa2e576 Research: chat-template tags in text the model reads; tips I15 and T33; an open decision
M5a task 04 stopped twice on the chat-template tags the check refuses.
The first time, the model read them in a file and took them for its own
template. The second time, it spelled them out in its own reasoning.

Measured on straylight: the think and tool-call tags are one token
whatever the tokenizer's flag. Inside any chat message, the turn
markers become control tokens, so a tool result can forge a system
turn.

Adds the rule to AGENTS.md, and proposes neutralizing the tags in
loopd as an open decision.

kyleandClaude Opus 5.5 f6dd7f1741 M4b review, and follow-up tasks 16 to 18
The review of tasks 01 to 15 goes in the implementer log, with the
device check against the owner's Mattermost. Three follow-ups:

- 16: the refusal without a runner says why.
- 17: the texts that say who asked.
- 18: approvals from non-approvers in direct or group messages go to
  an approver.

Task 14's given serve_approvals.rs becomes serve_approvals.rs-task14,
since task 18 hands over a new version. Tip T31 is added.

kyleandClaude Opus 5.5 af7137dafe M4b task 15: fix task 14's dropped refusal reason, as a listed edit
Task 15's session found the defect in serve/approvals.rs, a file it did not list, and stopped.
The comment in task 14 used one name for two values; task 14's tests could not reach it. Lesson T30.

kyleandClaude Opus 5.5 2d415fc1be Plan M4b: approvals over Mattermost in 15 tasks
Skeletons for new files only; existing files change by exact edits, so no task overwrites an
earlier one's work. Each task's tests were run against a reference built in order from master,
with the gate at each step and mutations checked; each skeleton compiles and fails its tests, and
every todo!() comment was read beside its signature. The reference is kept off this machine.

kyleandClaude Opus 5.5 02d5e25046 M4a task 14: post's comment was now_ms's; fixed, and every comment read
A script that rewrote the skeleton's comments put `now_ms`'s text on `post`. Task 14's second
session saw the contradiction and deliberated until cut off. Both comments are fixed, each
todo!() comment in tasks 14 and 15 was read beside its signature, and both tasks now say to stop
and quote a comment that does not fit. Lesson T29.

kyleandClaude Opus 5.5 564875f0a0 M4a tasks 14 and 15: glue written, calls tabled, after task 14's session wrote nothing
Task 14's session read the crate to learn the APIs and planned `connect` in prose until it was
cut off. `connect`, `Gateway::new` and `catch_up` are now written; the task lists every call with
its signature and makes the copy and the failing test the first actions. Task 15's comment is
rewritten one step per item. Both checked to fail, then pass when filled from their comments.

kyleandClaude Opus 5.5 02681069b6 M4a tasks 08 and 09: finer skeletons after task 08's session wrote nothing
Task 08's `header` was one todo!() with a dozen branches; Ornith planned it in its head until the
turn ran out (tip T25). `next_message` and `header` are now written as glue over seven small
helpers, and task 09's `poll` over four, each checked to pass when filled from its comments.

kyleandClaude Opus 5.5 0339dc13b2 Plan M4a: gatewayd in 15 tasks, with skeletons and given tests
Each task's tests were run against a reference at its end state; the end states were replayed
from master in order with the gate at each step (650 to 762 tests); each skeleton compiles
against its tests and fails them. The reference is kept off this machine. Lessons T27 (every
wait in a test has a limit) and T28 (mutate the reference before hand-over) come from this work.

kyleandClaude Opus 5.5 a57a1305e7 M3b plan: follow-up tasks 14 to 17 for the review's lower findings
14 moves the pipe handling out of container.rs (a pure move, replayed on its
own); 15 starts threads with Builder and bounds output collection with a 2 s
grace period; 16 escapes container errors in the log and fixes two texts; 17
fixes toolkit's thread start, casts and the egress-proxy form. Each checked
against a reference, which is not kept. Tips T24 to T26 from this run.

kyleandClaude Opus 5.5 b426ca1958 Specify and plan M3b: the runner and the tools
A draft spec for the owner's review and 13 offline tasks with their given
tests: shared tool arguments and host rules in proto, the sealed fetch
target (M3a finding 14), the toolkit tools and SOCKS5 egress proxy, and
brokerd's [runner], podman argument lists, runtime and proxy lifecycle. Each
task's tests were run against a reference at that task's end state (560 to
638 tests, clippy clean); the reference is not in the repository. Adds the
runner-unavailable runbook entry and tip T23 (ETXTBSY in script tests).

kyleandClaude Opus 5 22a26aad45 Review M3a: accept with fifteen findings; record the server change
Two medium findings in the audit writer (a startup panic on a record-less
log file, and a log-name filter that disagrees with bxctl's), one in the
missing runbook pointers for startup failures, and twelve low ones. Lessons
I14 and T21, T22; two new AGENTS rules; m3a's T18 renumbered to T20 so
master's T18 and T19 survive the merge.

kyleandClaude Opus 5.5 0a9df81fe4 Record the resolution of the M3a "fsync stall": a macOS socket rule
The handoff and the two stopped rows blamed an environment fsync stall.
The cause was macOS refusing socket options after the peer closes, in
loopd and in the brokerd test client. The handoff now opens with the
resolution, the log gets a review note (the stopped rows are kept as
written), and the lessons gain I13 (measure what you blame, and name
the machine) and T18 (the gate must pass on Talos and on the Mac).

kyleandClaude Fable 5.1 1008dce351 Decide where reference implementations are still written
From M3a: a full reference for the audit chain and the approval
handoff, the property test's oracle for policy, and compiling
skeletons for everything else. Each plan records what its checks
exposed, and that record decides what the next milestone gets.
Tip T17; the spec's handover section says which check each module has.

Also restores three / examples that an editing tool had decoded
to a plain slash.

kyleandClaude Fable 5.1 2be8581a0c Review M2b: accept with one follow-up task; record lessons
All ten tasks pass the checklist, the gate, the audit and the device
checks, including a four-turn conversation with a loopd restart and no
cache loss. Reading and probing found four low defects: the busy guard
is released before the final frame on the main path but not on the
three error paths, its Drop skips a poisoned lock, an unreadable
core.md is treated as missing, and bxctl's interactive loop exits on a
failed turn. Task 11 carries the fixes with two new tests, checked
against a fixed copy of the branch.

The Model column is filled in (all Ornith) and one malformed row is
repaired. Two rules are promoted to AGENTS.md.

kyleandClaude Fable 5.1 58c7738721 Review M2a: accept with two follow-up tasks; record models and lessons
The branch passes every check, including make verify-device on
straylight and repeated timing runs under load. Reading and probing
found that inferproxy does not pass an upstream close on to a client
that is still sending, and that the chunked body reader delivers a
stream only when the caller's buffer fills or the stream ends. Both
were also gaps in the tasks and tests, so tasks 14 and 15 carry the
fixes with new tests checked against the reference.

The Model column is corrected: tasks 04 to 06 and 08 to 13 were Ornith.
Lessons gain four implementer tips and five task-writing tips; three
rules are promoted to AGENTS.md.

tips for the implementer and for task writing, each tied to its
evidence, with a column to record whether the defect comes back.
