Initial implementation of mc-proxy

Layer 4 TLS SNI proxy with global firewall (IP/CIDR/GeoIP blocking),
per-listener route tables, bidirectional TCP relay with half-close
propagation, and a gRPC admin API (routes, firewall, status) with
TLS/mTLS support.
