Found in the M5b device check on straylight: the self-test asked about
each of the 33 tags and each neutralised form, 66 requests, one
connection each, and inferproxy's token bucket (burst 10) refused the
ninth, so loopd would not start. Now it asks about all the tags at once,
one per line, and then all their neutralised forms: the answer to the
first must hold each tag's recorded token in order, the second none of
them. Each tag is a special token, split out before anything else, so
the lines between cannot change it; and a neutralised form is only
dangerous as a tag's own token, so that is what the second answer must
not hold. The errors name a tag by its id, as before.
The fake tokenizer reads text with several tags in it; the tests cover
each tag as ordinary tokens, as another listed or unlisted token, a
duplicated token in the middle and at the end, a form holding each tag,
and both requests failing. Spec section 8 and the runbook entry follow.

Found in the M5a review: task 13 renamed core-memory-unreadable to
memory-unavailable but kept its body, which still described
memory/core.md, and appended a memory-index-damaged entry of its own that
names a pointer memoryd never prints and leaves out `bxctl reindex`. Its
log row says both fragments were copied. Both entries are now the plan's
fragments, verbatim.

A draft spec for the owner's review and 13 offline tasks with their given
tests: shared tool arguments and host rules in proto, the sealed fetch
target (M3a finding 14), the toolkit tools and SOCKS5 egress proxy, and
brokerd's [runner], podman argument lists, runtime and proxy lifecycle. Each
task's tests were run against a reference at that task's end state (560 to
638 tests, clippy clean); the reference is not in the repository. Adds the
runner-unavailable runbook entry and tip T23 (ETXTBSY in script tests).

From the independent review of task 23. A torn last line followed by an empty
later file had its recovery written into the later file, which broke the
chain for good; the line is now ended in its own file. The log-name rule
takes months 01 to 12 and days 01 to 31 only. [approvals] ttl_ms is limited
to a day, the longest loopd waits after a pending frame. Running out of file
descriptors or memory pauses the listener instead of stopping brokerd (the
errors the previous fix skipped do not occur on Linux). args.rs's doc fixed.

brokerd makes a socket's directory 0700. With a socket directly in / it would
chmod /, and through a symbolic link it would change the link's target. Both
are now refused at start with #brokerd-start-failed. Without the fix the link
case started and served, with the shared directory made private.

M3a review findings 3, 5 (the cast), 6, 7 (brokerd), 11. A config, directory
or socket failure at start now ends with docs/runbook.md#brokerd-start-failed,
and losing a listener with #brokerd-listener-lost; both entries are new.
Threads start through thread::Builder, so a refused thread is reported
instead of silently killing a listener; an aborted connection no longer stops
the daemon. brokerd reads args_os and keeps the config path as a path. The
"requester went away" result is recorded at the time it happens.

Every request is recorded, unreadable state is recorded as secret, a
refusal that cannot be recorded is an error, and the other cases the
brokerd reference settled. The audit-unavailable and
broker-state-damaged entries name the new messages; egress lists the
development calls to straylight.

brokerd internals: the ledger lock over the audit writer and session
state, the pending table's take-it-and-answer rule, sticky
audit_unavailable after a failed write, pure policy functions with an
Ask type that only redecide turns into a Decision, how a waiting
thread detects a lost connection without peek, fixed RunError text.
Approvals: bxctl chat fetches the block from brokerd by id, shows the
parsed arguments with invisible and bidi characters escaped, asks for
the approval id instead of y, and escapes model text. A tool_denied
event carries the reason to the owner.
Also: BrokerPort timeouts, tests for runbook pointers and concurrency,
threat-model notes, P14 for broker/sessions in the brief's State list.

Policy: a deny grant must apply at every taint; a result's label is
combined over all matching grants and the longest matched path wins
within a mode; a grant of / is invalid.
Audit: a recovered line need not fail to parse; the writer never goes
back to an earlier day's file; --accept-break verifies the whole log
and the break record's fields are all checked, with a seq counted from
lines; the Approval record carries the re-decision's grant and state;
calls with no Result are reported.

Split M3 into M3a and M3b, record the M3 design decisions, propose P13
(tool image built by Nix, named by digest), and add docs/runbook.md with an
entry for every fail-closed state, including loopd's existing ones.
