- Remove outdated `undo-state.md`
- Add two code quality/optimization reports that were used to guide previous work:
  - `code-report.md` (optimization)
  - `code-report-quality.md` (stability and code health)
- Add `themes.md`.
- Update undo system docs and roadmap.
